Trochanteric Bursitis

In the clinic this week I have seen and treated a couple of people who are training for marathons, half marathons and even ironmans. After gradually increasing their training, one person in particular began to experience hip pain.

One of the most common cause of hip pain is trochanteric bursitis. This is inflammation of the bursa which is located on the femur as it forms the hip joint (see infographic).

What is a bursa?

A bursa provides a cushioning to reduce the friction between a bone and large tendons. When the bursa sac becomes irritated or inflamed, it leads to pain or tenderness on the outside of the hip. As symptoms progress, this pain can spread down the thigh, buttocks, groin and sometimes into the lower back.

The most common cause of trochanteric bursitis include;

  • Trauma (direct from a fall or impact)
  • Repetitive use (running, climbing or increasing activity)
  • Congenital leg length discrepancy
  • Standing for prolonged periods

Symptoms may include;

  • Sharp or burning sensation in the hip
  • Buttock pain that spreads down the thigh or to the low back.
  • Pain laying on the affected side
  • Walking with a limp
  • Pain on activities; running, walking up hill or up stairs

After examination, we looked at any underlying gait and posture abnormalities which may be placing extra pressure on the hip.
Using a number of Osteopathic techniques such as massage, myofascial release, hip articulation and manipulation, not only at the hip joint, but the lumbar spine, pelvis, knee and foot to decrease pain and improve overall function together.

The management of trochanteric bursitis includes

  • Rest
  • Avoid laying on the affected side
  • Avoid exercising on an incline (stairs, hills)
  • Stretching before and after exercise
  • Strengthening muscles (to reduce the load on others)

Generally, no invasive procedure is required as most people respond well to conservative treatment. The prognosis of this condition is typically 4-6weeks, depending on the severity and underlying cause.

After a few sessions of progressive treatment and specific exercises, we are now pain free and back to a slightly altered training plan, with improved function overall.
